David’s journey: diagnostic uncertainty in practice

David, 61 — six months of loose stools and bloating. Bloods, coeliac screen, calprotectin and FIT all clear. Standard IBS treatment hasn’t touched his symptoms. Microscopic colitis and bile acid diarrhoea both remain possible.
